The company, which traded as SET Solutions, claimed for 40 hours of training even when an education course only ran for one day, the Victorian government training department's review found.
Victoria's Department of Education and Training brought in EY for a review, which handed down findings in August 2016. "The EY report shows a pattern of problems in other aspects of SET Solutions' record keeping and these errors should have been identified through robust internal quality controls and external benchmarking."
