The Malaysian Institute of Road Safety has been directed to immediately conduct a thorough investigation to determine the cause of the incident, said transport minister Anthony Loke.

“This is a tragic and regrettable event that should never have occurred. Action must be taken. The land public transport agency , together with the road transport department will conduct an audit of the company. If they fail or are found non-compliant with safety measures, their license will be suspended,” Loke said.

The incident occurred on the morning of Wednesday, April 13, at the traffic light-controlled intersection of Jalan Kebun Sireh and Jalan Bukit Tengah, when the deceased, Lee Zi Rou was driving alone in her car, waiting in line. Approaching from the opposite direction, the trailer lorry turned left towards the lane adjacent to Lee, when the container detached from the trailer, landing on Lee’s car. Lee succumbed to her injuries at the scene.
